Playground Swing Failure


Metallurgical failure analysis was requested of a playground swing set chain which failed, resulting in injury to a child.  Examination revealed that the design of the swing seat caused the s-link that joined the seat to the chain to wear and eventually break, releasing the swing seat and the swing user.
