Tractor Trailer Axle Failure

A metallurgical failure analysis of the failed tandem wheels and axle was requested after the driver side tandem rear tires of a tractor trailer failed at the axle, fatally injuring a pedestrian.

The failure analysis revealed that the wheel bearing failure was the result of lack of wheel bearing lubrication and/or improper maintenance.
