Accidents
Plant accidents occur at facilities which process chemicals, oil and gas, petrochemicals, manufacturing plants and other industrial
locations. When an accident or
plant incident occurs, rapid response accident investigation (FRAI) is extremely important.
Rapid response accident investigation and metallurgical failure analysis can often result in correct answers to "why" and
