Water Faucet Failure

Metallurgical failure analysis was requested after a water faucet failed, causing a water leak in a residence. The failure resulted from a split valve retainer nut. Examination of the split valve retainer nut revealed that there was a sharp filet radius inside the nut that initiated a fatigue crack causing the nut to fail. The sharp filet radius was determined to be due to a design or manufacturing defect.
